com) Position Description SCHOOL AND HEALTH COMMUNITY LIAISON Evergreen School District seeks a School and Health Community Liaison to join one of the leading K-8 school districts in the state of Montana.
Evergreen's mission is to enable students to become responsible citizens and lifelong learners by ensuring quality instruction in a nurturing environment.
The School and Health Community Liaison will work to connect with all families in the district and work closely with clinic and District staff to identify specific students and families who may benefit from utilizing additional health and school services offered in the District.
This position reports directly to the Special Services Director.
Preferred Minimum
Qualifications:
Bachelor degree preferred Three years of experience working in a school or social services environment Current knowledge of modern office practices and procedures Job
Responsibilities:
Develop priority list of families who need additional support and services Collaborate on student and family needs Follow up with students accessing services Determine improvement of student outcomes Develop and schedule flexible meeting times with families Meet with families and students Track evaluation evidence for grant Knowledge, Skills and Abilities:
Ability to perform clerical work and operate office machines Ability to use good written and oral communication skills Ability to use good word processing skills Ability to work with people and communicate clearly Ability to manage time effectively Ability to use good organizational skills and file management skills Ability to use good grammar and spelling Ability to use accuracy with numbers and documentation Ability to maintain confidentiality (strong ability) Ability to be well-organized and multi-task Ability to collaborate with multiple staff, agencies, and families Physical Demands:
Works inside a District building in a controlled, well-lit environment Work in family homes or off site environments Includes travel in personal vehicle within Flathead County Evaluation:
Performance to be evaluated annually by the Special Services Director Terms of Employment:
Hourly, 187 day contract year, Remuneration contingent upon preparation and experience Health insurance, leave, and retirement provided Work schedule/hours can be modified as determined by supervisor Note:
