Specific Duties Include:
Practice duties in accordance with the Social Work Code of Ethics.
Maintain a positive and productive working relationship with residents, families, staff, and supervisor.
Understand nursing home structure and current regulations, and these regulations' impact on individuals, families, and staff.
Build a healthier workplace and improve employee wellness with global mental health management solutions Work with members of the Nursing, Recreation, Environmental Services, and Rehabilitation departments on a routine basis.
Counsels and assists employees with issues affecting job-related performance and personal weel-being Ensure that residents are interviewed at least quarterly regarding their adjustment to nursing home placement, adaptation to physical and cognitive losses, and current needs.
Assist residents in locating and utilizing appropriate community resources and in meeting personal needs for clothing, aides, and incidentals.
Daily tasks may include employee education, short-term counseling and organizational assessments.
Respond to resident, family, and staff requests for assistance in solving problems as they relate to the social and emotional well-being of residents.
Know treatment options, including behavior approaches, validation, cognitive restructuring, group process, family interventions, and medication.
Know community and specialized resources, such as Medicaid, Medicare, Advanced Medical Directives, Hospice, resident empowerment groups, Alzheimer's family support groups, and committees; provide resident and staff support regarding these groups, as needed.
Demonstrate a thorough understanding Documentation:
Meet documentation requirements, using current computer technology.
Maintain understanding of documentation requirements, including Minimum Data Set (M.
D.
S.
) and task-centered casework; ensure that weekly M.
D.
S.
documentation requirements are met within required timeframes.
Ensure that psycho-social progress notes are completed in a timely fashion and maintain ongoing documentation of services provided.
Communication:
Effectively work and communicate with a wide range of individuals from all socio-economic groups, including geriatric guests/residents and those with special needs.
Communicate on a regular basis with Admissions and Care Team members regarding guest / resident / family issues, as needed, whether verbally or in writing.
Communicate with supervisor on a routine basis.
Participate in training and/or meetings that help facilitate the departmental communication process.
This position may interact with:
o Guests/residents, family members, and/or designees o General public o Immanuel administrators, directors, managers, supervisors, staff o Medical service providers, including physicians and pharmacists o Other service providers or representatives o Federal, State, Local government representatives o Members of the media Master's degree in the human services field, including but not limited to sociology, special education, rehabilitation, counseling, or psychology preferred, but at a minimum a Bachelor's degree At least one (1) year experience working directly with individuals in a health care setting required At least three (3) years' experience working in long-term care preferred The incumbent must be at least 18 years of age.
Required vision abilities include close up and distance, peripheral vision, depth perception, and ability to adjust focus.
