RN Resident Care Manager- The Retreat

DescriptionImmanuel Lutheran Communities is currently hiring for RN Nurse Resident Care Managers (RCM) for its Skilled Care Center in the Retreat Rehab Unite.
As a RCM, the position will also perform secondary duties and responsibilities according to Unit assignment as follows:
- Retreat West/Nurse Educator- Retreat East/Wound NursePurposeProvide professional nursing leadership, professional nursing care and clinical oversight to the patient/resident direct care team on the unit.
The leadership shall be rendered in an effective manner that is consistent and aligned with the (a) core values, mission, ethics and the Immanuel Spirit of Immanuel Lutheran Communities, (b) the policies and procedures of Immanuel Lutheran Communities/Immanuel Lutheran Skilled Care Center and (c) standards of care and regulations per regulatory agenciesLeadership Job Duties Human Resources & Supervisory Responsibilities Supervise staff assigned to the Unit.
Create a supportive department culture by fostering individual motivation, coaching and training for optimum job performance.
Collaborate with Human Resources to ensure each employee has the required and appropriate license and certification(s).
Give direction without conflict.
Anticipate staff problems and take preventive measures.
Analyze and respond to employee turnover taking appropriate steps to maintain high employee morale and satisfaction.
Educate employees on timekeeping policies and procedures to ensure accuracy.
Responsible for annual performance evaluations with employees on or before their due date for all employees supervised by the Resident Care Manager Plan work schedules and assign work to staff to ensure adequate service and coverage.
Decision Making/Judgement Analyze problems or procedures, evaluate alternatives and select best course of action; use of logic and commonly accepted practices in decision making to ensure timely effective solutions.

Assist the Director of Nursing in the follow through of annual departmental operations and budgets.
Monitor staffing levels and low census plans and make appropriate adjustments.
Demonstrate cost containment with all resources.

Provide oversight of the nursing staff and patient/resident care including staffing, orientation, education, policy/process compliance and overall quality of patient/resident interaction and care.
Create an environment of collaboration and mentorship.
Manager is expected to adjust schedule to include periodic night shift presence.
Manager will also support/assume nursing duties in extreme need situation or as operationally required.
Support customer service guidelines by developing patient/resident and family relations, supporting the care team to provide exemplary customer service and demonstrating proactive service recovery.
Initiate, coordinate and oversee the Resident Assessment Instrument (RAI) process on his/her unit.
Completes the nursing elements of the Minimum Data Set (MDS) 3.

Implement plans of care or assist other licensed personnel in implementing plan of care.
Facilitate care conferences in LTC, attend when necessary care conferences in TCU.
Coordinate and communicates with the interdisciplinary team.
Initiate and coordinate processes that will maximize reimbursement and assure compliance with federal and state guidelines.
ARDs optimize the PDPM System.
Facilitates and/or participates in weekly Utilization Review meetings and monthly triple check.
Adhere to State and Federal rules and regulations concerning delivery of resident care and assures that nursing services are provided and documented accordingly.
Evaluate nursing care delivery by administering resident care audits and perform focused rounds in the unit/facility to ensure that the safety, comfort and needs of patients/residents and families are provided in a timely, courteous and professional manner.
Maintain a visible and participatory presence on the unit and address needs or concerns in a timely manner.
Participate in the development and concurrent review of policies and procedures.
Monitor unit productivity standards, low census needs Maintain adherence of nursing practice on the unit according to the Montana State Nurse Practice Act, the State Board of Nursing regulations, the regulatory standards governing Immanuel Lutheran Communities and the application of the standards to the policies and procedures.
Participate in the safety, quality, and risk management programs to improve structure, process and outcomes of care.
Participate in organizational committees/task forces as assigned Assist Nursing Home Administrator with the preparation for and collaboration with other team members prior to and during the CMS and long-term care survey.

Current Healthcare Provider CPR certification.
Bachelor degree in Nursing preferred, but is not required.
A minimum of 2 years of experience in a nursing leadership role.
A minimum of 3 years long term care experience.
Will consider combinations of experience and education.
Possess knowledge of the health care environment, long term care regulations, and care delivery systems as they apply to the clinical practices on the unit.
Ability to employ critical thinking skills, decision making and professional judgment.
Experience with nursing information systems, Medicare billing, and PDPM.
